ABSTRACT

Objective: In this study, we aimed to determine the effects of the early phase of the COVID-19 pandemic on the services in Community Mental Health Centers (CMHCs) and on the patients followed up by the CMHC. In this direction, the clinical data of the patients and CMHC services were compared with the same period of the previous year. Method: Three CMHCs' data within the first three months following the first COVID-19 case report in Turkey (11.03.2020- 11.06.2020) were compared with the same period of the previous year retrospectively. Sociodemographic data, the number of admissions to the emergency psychiatry outpatient clinic, and the number of admissions to the psychiatry service of 2008 patients followed-up by CMHC were recorded as well as clinical parameters of inpatients and the quantitative values of the services provided by CMHCs. Results: The number of patients presenting to the emergency psychiatry outpatient clinic and admission to the inpatient psychiatry unit significantly decreased in the initial phase of the COVID-19 pandemic (p < 0.001). While a narrowing was observed in psychiatric hospitalization indications, face-to-face CMHC services decreased. Conclusion: The COVID-19 outbreak has created difficulties in preventing and treating COVID- 19 infection in individuals with severe mental illness, as well as difficulties in safely handling patients' existing mental illnesses. Considering the experiences from the early period of the pandemic, for current and future needs, the continuity of CMHCs should be supported and efforts should be made to provide digital services such as telepsychiatry applications when necessary (PsycInfo Database Record (c) 2022 APA, all rights reserved) (Turkish) Amac: Bu calismada COVID-19 pandemisinin baslangic doneminin, Toplum Ruh Sagligi Merkezler'indeki (TRSM) hizmetlere ve TRSM hastalarina etkilerinin belirlenmesi amaclanmistir. Bu dogrultuda, TRSM'de takip edilen hastalarin verileri ve TRSM hizmetleri onceki yilin ayni donemi ile kiyaslanmistir. Yontem: Turkiyedeki ilk COVID-19 vaka bildirimini izleyen uc ay icerisindeki (11.03.2020- 11.06.2020) uc ayri toplum ruh sagligi merkezinin ve bagli olduklari merkez hastanenin verileri retrospektif olarak incelenerek onceki yilin ayni donemi ile kiyaslanmistir. Takipleri bu merkezlerce yapilan toplam 2008 hastanin sosyodemografik verileri, acil psikiyatri poliklinigine basvuru ve psikiyatri servisine yatis sayilari, yatarak tedavi goren hastalarin klinik parametreleri yani sira TRSM'ler tarafindan verilen hizmetlerin niceliksel degerleri kaydedilmistir. Bulgular: Hastalarin acil psikiyatri poliklinigine basvuru ve psikiyatri servisine yatis sayilari pandeminin akut doneminde anlamli olcude azalmistir (p < 0,001). Yatis endikasyonlarinda daralma gozlenirken, yuz yuze verilen TRSM hizmetleri azalmistir. Sonuc: COVID-19 salgini, agir ruhsal rahatsizligi olan bireylerde COVID-19 enfeksiyonunu onleme ve tedavi etmedeki gucluklerin yani sira hastalarin mevcut ruhsal rahatsizliklarinin guvenli bir sekilde ele alinmasinda da zorluklar ortaya cikarmistir. Salginin erken doneminde yasanilan deneyimler dikkate alinarak mevcut ve gelecekteki ihtiyaclar icin TRSM'lerin sureklilikleri desteklenmeli ve gereginde telepsikiyatri uygulamalari gibi dijital hizmetlerin sunumu icin caba gosterilmelidir. ABSTRACT

Objective: The aim of this study is to determine the sociodemographic characteristics and psychiatric diagnoses of children and adolescents who refered to the psychiatry outpatient clinic during the COVID-19 pandemic. Method: This study enrolled the patients who were presented to the Child and Adolescent Psychiatry outpatient clinic between September 2019 and March 2020 and between September 2020 and March 2021. The patients were scanned retrospectively with the Nucleus outpatient clinic system. The data were analyzed with SSPS 21.0. Age, gender and psychiatric diagnosis of patients Results: were examined. This study enrolled 1157 patients with 10.8 mean age and 61.5% male participants. 53% (n = 622) of patients was diagnosed before pandemia and 46.2% (n = 535) of patients was diagnosed during pandemia. Before pandemia: patients were diagnosed with Attention Deficit Hyperactivity Disorder (ADHD) 40%, Anxiety Disorder (AD) 16.1%, Autism Spectrum Disorder (ASD) 5.1%. During pandemia: patients were diagnosed with ADHD 27.3%, AD 19.3%, ASD 5.6%. During the pandemia the number of female patients increased (p = 0.017), mean age of patients decreased (p = 0.035), the diagnosed ADHD rates decreased (p < 0.001) and the diagnosed AD (p = 0.024), Major Depressive Disorder (MDD) (p=0.001) rates increased. Conclusion: In our study the diagnosis of AD and MDD during pandemia increased and the diagnosis of ADHD decreased. Because of the online education during pandemia, number of ADHD patients decreased and increasing in diagnosis of AD and MDD might caused by phsycial and social limitations. In this period it is important to follow and treat the patients that already have psychiatric disorder like ADHD. Bulgular: Calismaya 1157 hasta dahil edilmistir. Hastalarin yas ortalamasi 10,8 yil ve %61,5'i erkek cinsiyetteydi. Hastalarin %53,8'i (n = 622) pandemi oncesinde, %46,2'si (n = 535) pandemi sonrasinda basvuru yapmistir. Pandemi sonrasi donemde, pandemi oncesi doneme gore basvurular kiz cinsiyet yonunde artmistir (p = 0,017) ve basvuranlarin yas ortalamasi azalmistir (p = 0,035). Pandemi oncesinde basvuranlarin %40,0'i Dikkat Eksikligi ve Hiperaktivite Bozuklugu (DEHB), %16,1'i Anksiyete Bozuklugu (AB), %5,1'i Otizm Spektrum Bozuklugu (OSB) ana tanisi alirken, pandemi sonrasi basvuranlarin %27,3'u DEHB, %19,3'u AB, %5,6'si OSB ana tanisi almistir. Pandemi sonrasinda pandemi oncesine gore DEHB tanisi alma sikligi azalmistir (p < 0,001), AB (p = 0,024) ve Major Depresif Bozukluk (MDB) (p = 0,001) tanilarinda artis gorulmustur. Sonuc: Calismamizda pandemi sonrasi AB ve MDB tanilarinin artis gosterdigi, DEHB tanisinin ise azaldigi gorulmustur. Pandemi sirasinda uzaktan egitim surecine gecilmesi, DEHB belirtileri nedeniyle yapilan basvurulari azaltirken ortaya cikan fiziksel ve sosyal kisitliliklar AB ve MDB tanilarini arttirmis olabilir. Bu donemde DEHB gibi mevcut psikiyatrik bozuklugu olan hastalarin takiplerinin ve tedavilerinin aksatilmamasi uzun donemde gidisat icin onem arz etmektedir. ABSTRACT

The article briefs about the importance of telepsychiatry in child mental health practices. The COVID 19 pandemic which started at the end of 2019 has affected the whole world. Children with mental problems and their families have had difficulty in accessing hospitals and psychiatric clinics due to the concern with being infected with the virus. As a solution to this difficult situation one of the methods most frequently applied by the professionals working in the field of child mental health is telepsychiatry. Telepsychiatry, which means providing psychiatric support to people outside a clinic, office or hospital through videoconferencing or other tecnologies, has been used more commonly during the pandemic period. The pandemic has provided an opportunity to introduce innovation into mental health care. This and open up new possibilities for care.
